Do not use this form for objecting to a recommended ALJ decision. Take or mail the signed original to your local Social Security office, the Veterans Affairs Regional Office in Manila or any U.S. Foreign Service post and keep a copy for your records. If you do not agree with the decision or order an Administrative Law Judge (ALJ) made on your claim, you may ask the Appeals Council to review the ALJ's action. To do this, you may use this form or write a letter. If you are not sure this is the form you should use, the notice you received will tell you that to appeal the ALJ's decision or order you must request Appeals Council review. You must file your appeal within 60 days from the date you got the hearing decision or order.
