This form is used for making a data change for a previously issued, currently valid U.S. passport WITHIN ONE YEAR FROM DATE OF ISSUE.
Use this form:
When there is a change in identifying information, such as name-change due to marriage or court order;
In order to extend validity of certain previously limited passports;
or
In order to correct a Passport Services’ printing error.
If it is MORE THAN ONE YEAR since issue date, you must submit a DSP-82 or DSP-11 form.
For Printing Errors:
More than 30 days since passport was issued, submit the following:
Two new passport photos;
Letter explaining specific correction needed and date by which corrected passport is required;
Passport containing printing error;
and
A completed DSP-5504 Form.
Less than 30 days since passport was issued, submit the following:
Letter explaining specific correction needed and date by which corrected passport is required;
Passport containing printing error;
and
Completed DSP-5504 Form.
