Use this form to renew your passport providing all the information remains the same, i.e., no name changes. You can use this form: If you can submit your most recent passport; If you were at least 16 years old when your most recent passport was issued; If you were issued your most recent passport in the last 15 years; and If you use the same name as on your most recent passport or since your passport was issued, and have name changed by marriage or court order and can submit proper documentation to reflect the change.
