DSP-11 Application for Passport (First Time & Minors)

This is the official form needed to apply for a United States Passport.

Use this form if:

If you never had a U.S. passport;

If you are under age 16;

If your most recent passport was issued more than 15 years ago;

or

If your most recent passport was lost or stolen, you are also required to submit Form DSP-64.
